Case study:

Bromley Personal Health Budgets (PHBs)

Mr C was receiving a social care direct payment, when he was assessed as having continuing healthcare needs. He wished to continue his existing support package and was referred to Vibrance to support the transition in funding.

The process began with writing a support plan to ensure that his clinical and social care needs were being met and that he retained the flexibility that he had previously. We shared with him our PHB support planning template and Mr C took the lead in the writing of his plan and identifying his personal outcomes:

  • I will have more control over my care -  improving my quality of life and increasing my life expectancy
  • I will continue living at home in my own environment
  • I will visit places of interest to me regularly and have opportunities for respite care
  • I will have more positive experiences of care from the hospital environment and the time I spend unnecessarily in hospital will reduce

The plan has now been submitted for agreement by the CCG Panel, and Mr C has shown an interest in providing peer support to others who are considering taking up a PHB.
